Puncture biopsy of heart tumors under computer tomography navigation

Abstract

The  aim – to enhance cardiac tumor biopsy techniques in order to minimize complications associated with invasive diagnostic procedures.
Materials and methods. This article presents clinical experience of a novel approach for the diagnosis of cardiac tumors predominantly located in right heart chambers. The method involves CT-guided percutaneous biopsy. It was employed in four patients with primary involvement of the right ventricle, right atrium, pericardial free wall and pulmonary artery – structures closely adjacent to the chest wall.
Results. The proposed method was implemented in patients diagnosed with various malignant cardiac tumors. The use of CT-navigation was found essential for accurate localization of the tumor in relation to cardiac anatomy and its involvement in the pathological process. Stepwise CT-guided needle insertion enabled safe tissue sampling, avoiding trauma and complications associated with open surgical biopsy.
Conclusions. Due to the close anatomical relationship between cardiac tumors and vital heart structures, tissue sampling carries a risk of injury. Therefore, precise, cautious manipulation is required to avoid vascular or structural heart damage. Percutaneous biopsy of tumors adjacent to cardiac structures should be performed in a stepwise manner under CT-guidance.
